CHRONIC DISEASE RECALL PROCESS FOR BUTTERCROSS AND ILCHESTER
INTRODUCTION
The aim of this process is to ensure that all patients with a chronic disease are reviewed once a year as a minimum.
Chronic conditions review invite
The Health Centre is monitoring patients with long term chronic diseases such as Asthma, COPD, Diabetes, high blood pressure, circulatory problems, heart conditions, kidney disease, Stroke/TIA or past history of mental illness.
We would like to invite all patients with chronic conditions an annual review. This will consist of an appointment with one of our healthcare team for a basic review and test (blood pressure, blood tests, weight & urine testing dependent on condition). If your condition requires a follow up with a nurse or GP this will be arrange once you have had this first appointment.
How we will help you manage your long term condition
Patients with long term chronic conditions require regular checkups with their GP practice to ensure that they are on the most effective treatment for their condition. By having these regular checkups we can help you to prevent any problems in the future and hopefully reduce your risk of hospital admissions.
By running our birthday review process we aim to be calling all patients in once a year, regardless of how many conditions you have, and take a more person centered approach to your care. As we introduce this system over the next 12 months you may find your next review is slightly earlier than expected. This will be to bring our reviews in line with the month of your birth so that we can be confident in contacting all of our patients.
